Facility: Report Card Settings

In this article, we'll show you how to customize Report Cards you can send to your clients. These can be configured in your Settings → Client Settings → Report Cards.

Report Card Settings

Report Cards can be enabled in the Report Cards section of your Settings.


Facility supports nearly 100 different icons. Icons should be related to your question. For example, the "Bowl" icon may be related to a question about how much food a pet ate. Question types can be simple "Yes/No" questions, a "Count" question (where you can enter a number), or a customizable "Multiple Choice" question.

After creating your questions - you can re-order them by dragging and dropping the question. You can also edit (and delete) questions as well, by clicking "Edit" next to a question:


Creating Multiple Report Card Templates

Facility supports using multiple Report Cards. For example, the list of questions will likely vary based on whether a service is for Daycare or Boarding. You can create a “Daycare Report Card” and assign this to the "daycare" services. Similarly, you can create a “Boarding Report Card” and assign it to your “boarding” service.


Initially, all the questions you add will be added to the "Default" Report Card. All services will use this "Default" Report Card unless the "Is Default?" is selected on another Report Card:

To create an additional Report Card, click the "Create Report" button:

Enter the name of the new Report in "Report Title" and click "Add Report":
